Jazmin is the Chief Program & Strategy Officer at Partnerships for Community Health, where she leads strategy and program design to advance equity-driven outcomes across communities. She brings experience working with historically marginalized populations in both New York and New Jersey, with a focus on community voice, systems change, and population-level impact. Her leadership is guided by a deep commitment to birth equity, place-based strategies, and collective impact models that center the voices of those most affected by health inequity. As a mother of three, Jazmin approaches her work with urgency, care, and a vision for long-term change.
She holds a Master of Public Health from NYU and is currently a doctoral candidate in Education at the CUNY College of Staten Island.
