Transition and Teaming has helped and continues to guide a Local Community Center in Texas through ongoing growth, funding changes and evolution. Through collaborative teaming, the lives of people that we serve were transformed by being able to provide comprehensive services to meet the needs of the whole person. Staff were empowered to meet these needs through collaborative trainings that provided tools to be able to work with everyone, regardless of the person's cognitive, communication or functional abilities.
